Licensed Practical Nurse (LPN)

We are hiring for a private home health care company that is committed to delivering compassionate, professional, and personalized care to individuals in the comfort of their homes. Position Overview:
We are looking to hire experienced LPNs to join our team for both day and night shifts in our 24/7 home health care services as needed. As part of our team, you will have the opportunity to provide high-quality care and build meaningful relationships with patients and their families.

Responsibilities:

• Provide skilled nursing care to patients in their homes, following physician’s orders and care plans.
• Administer medications and treatments as prescribed.
• Monitor patient health status, record observations, and report changes in condition to the healthcare team.
• Provide wound care, catheter care, and other nursing procedures as needed.
• Communicate effectively with patients, families, and healthcare professionals.
• Educate patients and caregivers on proper care techniques and health maintenance.
• Document patient care accurately and in a timely manner.
• Adhere to safety protocols and infection control standards.
• Ensure a compassionate and supportive environment for patients and their families.

Qualifications:

• Current and valid LPN license in Georgia
• Minimum of 2 years of home health care experience required.
• Strong clinical skills and the ability to make independent decisions.
• Excellent communication and interpersonal skills.
• Ability to work autonomously and manage time effectively.
• Current CPR certification (or willingness to obtain).
• Reliable transportation and valid driver’s license.
• Compassionate, patient-centered approach to care.
